SMS Messaging Terms and Conditions

 

Purpose:

SMS communications are used for essential communication, including patient record setup, notices of service, alerts of patient conditions, updates, confirmations, and reminders related to services of patients and family member, and necessary correspondence with doctors, hospitals, clinics, insurances, Medicare, Medi-Cal regarding to patients’ needs and conditions.

 

Opting in:

A mobile user might opt-in by: Entering a phone number online; Sending a message with “START”, “SUBSCRIBE”, “AGREE” to our message service. Customers can also consent for the SMS communication by checking the consent checkbox on our online contact form. By opting in, you agree to receive informational messages (service/visit reminders, notifications, etc.) from SolisMed Home Health Care Inc.

 

Upon granting consent to receive SMS communications from our company, please be assured that your phone number and SMS consent will remain confidential. We will not sell or share your information with third parties or affiliates. Your phone number and consent will solely be used to facilitate communication with you and will not be disseminated for marketing purposes.

 

Opting out: 

You can opt out by texting “STOP”, “UNSUBSCRIBE”. After opting out, you can opt in again any time by “START”, “SUBSCRIBE”, “AGREE” to our message service. However, you are aware and agree that if you opt out of our message system, it will be your responsibility that medical service may be delayed or even canceled if we are not able to contact you or send you information regarding the services and health notices.

 

Message Frequency: 

The message frequency may vary depending on the patient’s status and need of the medical services. We will not send messages to you without proper reason or need. We only send messages when it is needed, for example, the notice and reminder of service, quality assurance survey and feedback, necessary information request.

Please be advised that the message and data rates may apply. We do not charge you any cost for the SMS communication. Please contact your mobile service provider for the cost and fee regarding to SMS.
